About Your Medical Consultation
Our goal is to provide you with the highest level of personalized care in an upbeat and supportive environment. Each visit is designed to create momentum toward health.
Your initial visit will include a 50 minute consultation with Dr. Wiese. Laboratory and other diagnostic testing may be recommended. The purpose and the cost of testing will be discussed with you at the time of your visit. Nutritional supplements are often recommended and we will help you find the highest quality products. Test results and how they impact your plan for health will be discussed at a follow-up visit.
Dr. Wiese is an out-of-network provider. We provide you with the appropriate forms and coding so you may file your claim. For your convenience, Visa, MasterCard, personal checks or cash is accepted. Payment is due on the day of service.
Medical Records
Previous medical records are helpful for creating a complete picture of your current health. You are responsible for requesting previous medical records from other physicians or health care providers. Bring a copy of your medical records with you to your first visit or have them mailed, emailed or faxed to our office prior to your initial visit.
Medical records will be released from our office only with your authorization.
